Do you know a special teacher?
We’re asking Toronto Star readers to nominate someone in the teaching
profession for the first-ever Toronto Star Teacher Award.
Submit your nomination to us, including the following information:
• First and last name of the teacher
• Where the association with the teacher took place (provide the name
of the school or teaching facility)
• Dates of the association with the teacher
• Your name and daytime telephone number
• Your relationship to the teacher (ie. student, parent of the student,
former student, colleague).
Then, in 250 (minimum) to 500 words, provide a personal account
explaining why this teacher deserves to be recognized with the Toronto
Star Teacher Award. This may include a brief description of his or her
qualities, teaching characteristics, specifi c examples of the teacher’s
efforts, dedication, mentorship, or the difference he or she has made to
your life or the lives of others.

Deadline for Nominations: Tuesday, July 1, 2009.
A Panel of Judges will select a Recipient, who will be announced in the
Toronto Star in October 2009.
RULES:
1. Nominee must be a certifi ed Ontario teacher at
the elementary or secondary level
2. Your story must have taken place between
September 2008 and June 2009
3. Teachers may not be nominated by a
member of their family
4. All nominations must be accompanied
